For proof of disability:
According to the SSA, a person with disability is unable to engage in any substantial gainful activity (SGA) by reason of any medically determined physical or mental impairment(s) which can be expected to result in death or which has lasted or can be expected to last for a continuous period of not less than 12 months.
A person with disability is:
- An individual who is aged, blind or has a disability as described under Title II of the Social Security Act.
- A person with an injury or illness that is anticipated to last for a long period of time causing long-term incapacitation.
- People who suffer from developmental disability (or related condition) or mental disability.
- Anyone who has mental health condition, substance use disorder or physical injury that required a residential level of care and who is now in the process of transitioning to the community.
- Persons who have been determined to have a learning disability according to policy adopted by Department of Human Services (DHS); and
- Individuals who have substance use disorder and are enrolled in a treatment programs or are on a waiting list for a treatment program.
- People who have been assessed to require assistance with communication, mobility, decision-making; or managing challenging behaviors due to a disability or a long-term or a chronic condition.
One of the following documents will be needed and must be signed by a qualified professional:
- Professional Statement of Need (PSN) (preferably, can be used as proof of both disability and assessment)
