HOW IT WORKS
Housing Consultation Services helps a person who has Medical Assistance to develop a housing focused person-centered plan that addresses their needs, wants, and goals as well as assisting a person to make informed choices in their housing transition or sustaining services provider
During Housing Stabilization Services Consultation stage, Ability Care Services consultant will perform a consultation to predetermine eligibility by verifying qualifications. Information such as applicant age, insurance information, housing status and documents verifying proof of disability and assessment to determine if a person is homeless or at immediate risk of homelessness will be looked at.
According to the DHS, housing instability is evidenced by the following:
- Homeless

Homeless, an individual or family is considered homeless when they lack a fixed, adequate nighttime residence.
- Currently transitioning, or has recently transitioned, from an institution

Currently transitioning, or has recently transitioned, from an institution or licensed or registered setting (registered housing with services facility, board and lodge, boarding care, adult foster care or community residential setting, hospital, Intermediate Care Facility for persons with Developmental Disabilities (ICF/DD), intensive residential treatment services (IRTS), the Minnesota Security Hospital, nursing facility, regional treatment center)
- At risk of homelessness, an individual or family is at risk of homelessness when:

At risk of homelessness, an individual or family is at risk of homelessness when:
The individual or family is faced with a situation or set of circumstances likely to cause the household to become homeless, including but not limited to: doubled-up living arrangements where the individual’s name is not on a lease, living in a condemned building without a place to move, having arrears in rent or utility payments, receiving an eviction notice without a place to move or living in temporary or transitional housing that carries time limits; or
- The person, previously homeless, will be discharged from a correctional, medical, mental health or substance use disorder treatment center and lacks sufficient resources to pay for housing, and does not have a permanent place to live; would be at risk of homelessness if housing services were removed

- At risk of institutionalization.

– If a person meets an institutional level of care or qualifies for thefollowing waivers:
- Brain Injury (BI)
- Community Access for Disability Inclusion (CADI)
- Community Alternative Care (CAC)
- Developmental Disability (DD)
- Elderly Waiver (EW)
A consultation is done by either of the following:
- Ability Care Services Consultant

A consultant will complete a Housing Focused Person Centered Plan with the applicant. (DHS-7307)
- Targeted Case Manager

A Targeted Case Manager will complete and send to us your completed Housing Focused Person-Centered Plan. (DHS-7307)
- Waiver Case Manager

A waiver case manager will complete and send to us your Coordinated Service and Support Plan (CSSP)
- Care Coordinators

A care coordinator will complete and send to us your Coordinated Care Plan (CCP).
All documents of proof of need of HSS will be submitted to Ability Care Services together with your referral form.
After housing consultation, all documents will be submitted to the DHS for eligibility. If the applicant is approved, the DHS will send approval letter to both the applicant and the transition and sustaining agency of applicant’s choice at the time of consultation. At this point Housing Stabilization Services Transition and Sustaining will follow.
